Foltsbrook Center for Nursing and Rehabilitation - The Social Worker is positioned to provide the new resident with a meaningful and comprehensive orientation to the facility. The Social Worker is an advocate for the resident and works closely with the Care Plan Team and functions in the role of liaison between the resident, family members and staff responsible for providing care and services to the resident.
