Description The Social Worker functions as a member of the interdisciplinary treatment team. The Social Worker provides individual, group and family counseling services. Functions as the primary clinician to a caseload of clients and is responsible for the case management discharge planning and necessary documentation related to their caseload. The Social Worker is responsible for assessments and hospital consults on an as needed basis. Participates in case conferences and staff meetings. Has extensive contact with referent agencies, such as Department of Social Services and the judicial system to collaborate in treatment planning. Acts as liaison and provides outreach to community agencies when needed. Works with special populations including traumatic brain injury, PTSD and chronic relapse. Qualifications A Masters Degree in Social Work(MSW), Mental Health Counseling (MHC) or related field. Two to three years experience providing counseling to chemically dependent and psychiatric patients required. Substance Abuse training (CASAC-T) or CASAC a plus. LMSW/LCSW/LMHC.
