We are looking to recruit a Qualified Social Worker, Older Peoples Team within the Social Care Industry. Our client has a well-structured and organised team that are looking to grow their staffing levels. The role offers flexible working hours for working parents and has a supportive team culture with regular supervision on cases. To be successful:
- Carrying out assessment's for Older Persons and their carers
- Obtaining multi-disciplinary assessment of need
- Care management
- Designing intensive care packages
- Carrying a caseload of service users
- Make recommendations for short-term care, intermittent care and permanent residential care where appropriate
- Self-directed support / personal budgets
For this Qualified Social Worker, Older Peoples Team you will need to be a Social Worker with a HCPC Registration and educated to Degree Level. You will also need to have the Eligibility to Work in the UK with previous experience working within an Adults' Older Peoples Team. Benefits of Working for Durham Council:
